The following forms are available for you to download and print.
All of the documents require 8-1/2 x 14 legal size paper.
Both types of applications require a $5.00 membership fee.
A letter of credit from a previous or existing utility supplier, or a $250.00 deposit is also required at time of application. Formal application for new membership must be made in person at MCREA headquarters.
